All stakeholders were not conscious of the safety risk and consequences

Once the EWP was deemed impracticable to be used as a protection device, there was no edge protection

Worker that fell made a small mistake that ended up in his death, this was not controlled or accounted for in planning

Poor Supervision by activities supervisor

EWP Scissor lift was not placed before the incident due to not being practicable

Fall arrest harnesses were not being utilised during the time of incident

Lack of other technology to enforce fall protection like the provision of safety nets

Lack of respect for the height of the work area

One person was responsible for making sure the roofing parts were straight and not to fall off the end

No provision for Safe Work Method Statements

No provision for company procedures of working at height

Contract was not adhered to, not putting up safety barrier for workers

No attention to the implementation or monitoring of hazard controls

No attention to the quality of safety provisions

No goals to adhere to the Laws, Regulations, Codes of practice or Australian standards

Failure of management to enforce the use of a safety barrier

Too much focus on profits, rather than the safety of the workers involved

Extremely poor understanding of safety laws in Australia

Tolerance of poor standards from management and higher stakeholders

Not a top down safety concern, or bottom up, nobody was evidently concerned

Focus was on profits rather than safety
