Contract was not adhered to, not putting up safety barrier for workers

Lack of respect for the height of the work area
One person was responsible for making sure the roofing parts were straight and not to fall off the end

EWP Scissor lift was not placed before the incident due to not being practicable
Fall arrest harnesses were not being utilised during the time of incident
Lack of other technology to enforce fall protection like the provision of safety nets
