Intense exertion caused strain on her muscle fibers, leading to damage to muscle cell membranes.

Why did this happen: Due to excessive exercise, Rachel ran out of ATP.
ATP has the role of kicking out calcium from the cytoplasm into the sarcoplasmic reticulum through SERCA when contraction is done. However, in its absence, it overstays its welcome.
Too much calcium for too long will lead to Apoptosis (cell death). With the cell dead, it can't hold onto myoglobin.
