Puritus is problematic
Tinea cruris
Tinea capitis

Expected outcomes
Patient reports no pain and no itching
Patient would get 6 to 8 hours of undisturbed sleep
No signs of infection
Vital signs normal
Skin colour and texture normal
Nursing intervention
Administer antifungal drugs as prescribed
Apply local antifungal creams
Maintain standard personal hygiene
Keep skeen clean and dry at all times

Disturbed sleep
Impaired skin integrity
Altered comfort
Itching and pain
Skin colour and texture changes
Evaluation
Skin colour is restored
No itching and pain
Vital signs are normal
No infection evident
Patient reports that they are sleeping well
