Monitor student performance and behavior.

Identify struggling students who may require extra support.

Implement classroom-based interventions to address student's difficulties.

Monitor the student's response to interventions.

Communicate concerns with parents and school support staff (like school counselor, psychologist, etc.).

Discuss student's difficulties and intervention strategies.

If the student continues to struggle despite interventions, initiate a referral for a special education evaluation.

Obtain parent's consent for the evaluation.

Conduct a comprehensive evaluation in collaboration with a multi-disciplinary team.

Identify the student's unique needs and determine eligibility for special education services.

Develop an Individualized Education Program (IEP) in collaboration

with the student's parents and other school personnel.

Define goals and appropriate accommodations/supports for the student.

Implement the IEP in the classroom.

Provide special education services and accommodations as stipulated in the IEP.

Regularly monitor student's progress towards IEP goals.

Conduct annual reviews and necessary revisions of the IEP.

Parent and Community Involvement:

Keep parents informed and involved in their child's educational progress.

Engage community resources and supports as necessary.
