HIGH ALERT MEDICATIONS (HAM) 2ND EDITION 2021
DEFINITION
High Alert Medications (HAM) are medications that are most likely to cause a significant harm to the patient even though it is being used as intended.
PURPOSES
i. To assist healthcare professionals in identifying high alert medications.
ii. To establish safeguards to reduce avoidable harm and the risk of errors with high alert medications in healthcare facilities \
iii. To accelerate the implementation of safe preventive strategies in all stages of the medication use process.
iv. To increase awareness of healthcare professionals on the importance of counterchecking in safe handling of high alert medications.
THREE MAIN PRINCIPLES
Reduce or eliminate the possibility of error
limit the number of HAM on hospital/ KK
limiting the concentration/ strength of medicines available)
Make errors visible
Counter Check medicines from two medical health workers
Always use 5 rights
Minimize the consequences of errors
MANAGEMENT OF HAMS
Should have HIGH ALERT MEDICATION label on HAM meds
Use HAM sticker or HAM envelope for the HAM meds
Review/ evaluate checklist by using the assessment form
Any changes of brand/ colour/ preparation of the HAM meds must inform users
All HAM meds will be dispensed and administered by using safe practices
All HAM meds should be counterchecked before dispense to patient
All equipment/ device use for HAM meds should be calibrated according to SOP
Identify and keep separately or apart HAM meds that are look alike and sound alike
