Formative assessments are used to gather evidence of what students are learning. Teachers can use the feedback obtained to adjust instruction.
Interim/Benchmark
Formal style testing used periodically through out the year. Information gathered is used to determine if students are on track.
Example: MAP Testing is done at the beginning, middle and end of the year to track student growth.

Diagnostic
Diagnostic assessments are used prior to learning to determine students strengths, weaknesses and help break any misconceptions.
Examples: KWL chart used before and after a lesson to determine what students know, want to know and at the end to determine what students learned.
Examples: Running Record used to help place students in the correct reading level. Used to know students strengths and weaknesses prior to reading.
Performance
Performance assessment is a form of assessment that requires students to demonstrate by doing what they have learned or are able to do by constructing an answer.
Example: Performing an activity, teacher presents an open ended questions students respond orally.
Example: Retelling a story, students read a book and in their own words retell the story.
