Service users not responding to invite letters.
Service users are not being seen therefore no data is provided for stakeholders.
HbA1c readings from GP at risk of becoming out of date results.
A team meeting was carried out and all 7 Health and Wellbeing Facilitators (HAWF) had the same issue.
Change needed to get service up and running.

Changed format of the letter.
Advised patients via the letter that the HAWF will contact them if no response after 2 weeks of sending the letter.
Will trial for six weeks.
Change leader nominated and weekly team meetings agreed to record progress.
Feedback forms were provided to service users to record any negative or positive comments

Feedback forms analysed.
Better response after speaking to service users via telephone.
Service started to evolve and service users being seen.
Data and figures are being sent off to secure funding.
PLAN
Adopt the new change.
Keep calling service users via telephone to secure appointments.
Weekly meetings were made permanent to boost moral and to voice any concerns.
Normalised the new procedure and embedded it into the workplace.
