MOA
- NAPQI - toxic metabolite of Paracetomol
- Usually conjugated to Glutathione and excreted
- In OD a larger dose of paracetamol converted to NAPQI, and glutathione depletes, leading to build up
- NAPQI attacks nucleophilic thiol groups causing mitochondrial damage and necrotic cell death
