clinical contemporary study: kroenke et al
aim
to assess the validity of a shortened version of a questionnaire based on DSM IV criteria as a measure of depression in a large scale population study
procedure
selected participants randomly and nationally
they contacted participants over the phone
ask pp to consider last 14 days with regard to specific symptoms and allocate points accordingly out of possible 24
10 or more out of 24 represents clinically significant moderate depression
the BRFSS also gathered info on quality of life
198, 678 people responded
61.6% were female, majority where white and non hispanic
18% of the sample had previously received a diagnosis for depression
12.3% had recently received a diagnosis for an anxiety disorder
after asking questions, a comparison was made between those respondents scoring 10 or more on PHQ8 and other measures such as depression status
results
high degree of concordance between those scoring below 10 and no diagnosis of depression
a large degree of agreement was found between measures of depression and scoring over 10
conclusion
thus suggesting that it is a good way to investigate large scale samples quickly and efficiently for example via a self report questionnaire (postal or web based)
when compared to other measures, PHQ8 yielded similar prevalence rates
evaluation
+standardised procedure
+ethical- omitting suicide question
+all trained interviewers
+useful practical applications
-sample limited to those with phones
-focuses only on symptoms of last 14 days may be measuring effects of events on respondents life rather than genuinely measuring depression
-social desirability
-only american adults- differences in culture and views of depression
PHQ8-9
PHQ9 was developed as usual structured psychiatric interview too time consuming and difficult to administer to large amounts of people. depression is common. quick assessment. also to check prevalence rates of depression in US
PHQ8 differs as does not include 9th question which assesses suicidal or self injurious thoughts as interviewers are not able to provide intervention over telephone
