7 steps of RCA

:black_medium_small_square:What are the business benefits to solving it?
- Collect data relating to the problem
- Identify what caused the problem
:black_medium_small_square:Identify as many causes as possible
:black_medium_small_square:Involve all key stakeholders
:black_medium_small_square:Use a suitable root cause analysis methodology such as 5 Why's or a Fishbone diagram
:black_medium_small_square:Prioritise the causes from the prvious step. Try to reduce the list further if you can.
:black_medium_small_square:Confirm the methodology you want to use.
- Identify solutions to the underlying problem
:black_medium_small_square:The aim here is to identify workable solutions to prevent the problem from happening again.
:black_medium_small_square:When identifying solution, bear in mind things like time, cost, capacity etc.
:black_medium_small_square:Who will implement the change?
:black_medium_small_square:When will the change be implemented by?
:black_medium_small_square:What does success look like?
:black_medium_small_square:How will the solution be monitored?
:black_medium_small_square:How will the new solution work/be embedded into existing business processes?
:black_medium_small_square:How will the impact of the solution be analysed?
Root Cause Analysis
RCA is about finding the origins of the cause of problems, then applying appropriate solutions, preventing it from happening in the future.
