Outcomes
Patient will verbalize acceptance of self and coping strategies for psychological needs by the end of the week.
Patient will demonstrate attempt to increase mobility by participating in physical therapy exercises by the end of the week.
Nursing Interventions
Nurse will assess skin frequently especially on the paralyzed side to maintain skin integrity
Nurse will reduce risk of future strokes by applying SCD therapy to the lower extremities.
Nurse will educate the patient on risk factors of strokes to prevent future strokes.
Nurse will assist patient to perform ADLs and encourage independence when assisting
Discharge Outcomes
Patient will be mentally and physically ready to discharge to a rehabilitation facility for building strength and mobility further.
Avoid
Diet with high fats, sodium and processed foods
Improper alignment and body positioning of the left extremities
