Definition of Acute Otitis Media
Infection that occurs in the middle ear, eustachia tube, antrum, and mastoid cells
Classification of Acute Otitis Media
Suppurative
Perforation
Hiperemia
Resolution
Occlusion
Risk Factors of Acute Otitis Media
Children are more at risk
Upper respiratory tract infection
Etiology of AOM
influenza Virus
Rhinovirus
Haemophilus influenza
Adenovirus
Streptococcus pneumoniae
Patofisiology of AOM
Bacterial infection of the middle ear results from nasopharyngeal organisms migrating via the eustachian tube. The anatomy of the eustachian tube in younger children is immature, typically being short, straight, wide (only becoming more oblique as the child grows), meaning infection is more likely.
Complication of AOM
Labirinitis
Brain Abscess
Meningitis
Classification of deaf
Sensorineural Hearing Loss
Mixed Hearing Loss
Conductive Hearing Loss
DIfferencial Diagnoses
otitis media with effusion (OME)
chronic otitis media (COM)
external otitis (otitis externa)
herpes zoster infection
Physiology of Hearing
Sound waves enter the outer ear and travel through a narrow passageway called the ear canal, which leads to the eardrum. The eardrum vibrates from the incoming sound waves and sends these vibrations to three tiny bones in the middle ear. These bones are called the malleus, incus, and stapes
Prognosis of AOM
The prognosis of otitis media is usually good with or without treatment, but varies based on the classification
Pharmacology treatment
Occlusion - Oksimetazoline 0,0025-0,05%
Hiperemia - Eritromicin,Azytromicin,Ampicilin
Suppurative - Oral Antibiotics and refer to ent specialist
Perforation - H2O2 3% 3 x 1, 4 drops (2-5 minutes)
Resolution - Observation
Education
Keep ears clean
Don't pick your ears
Avoid boarding and diving
Immediately go to the hospital if the disease gets worse
Prevention
Ensuring children get complete and routine vaccinations, including PCV
Keep children away from exposure to cigarette smoke and air pollution
Keep children away from public places where children may be exposed to ARI
