Bullying
Is there bullying in class? If a student is struggling with bullying, they might not want to come to class or participate, and try to 'hide' away.
Home life
PTA/Excursions
A lack of attendance to PTA days or excursions could be a social problem, or a homelife problem that might need investigating
abuse?
If there is abuse in the home, the student will be less concerned with test scores or even concentrating in class.

Behaviour
good or bad
Behaviour can be affected by home life as well as social aspects in the class room. In my experience, Sometimes the bad behaviour (or good) can be a result of outside factors.
Against all teachers?
Is the behaviour only apparent in one class or for one teacher? Does that teacher need to think about classroom management changes, or the way they are teaching?
concentration
If the student can't concentrate, or struggles to find motivation to try in class, perhaps there is an issue with the course, or with the teaching style

Classwork
Does student contribute
If the student is shy, or doesn't get on with their classmates, this would dissuade them from participating in group discussions or activities during class.
is work on time
If the work isn't on time, or not complete, there could be an issue with teaching methods, or checking the understanding of the students during class time. If the work is late, there could be outside factors that hinder the student's timekeeping.

Test scores and grades will suffer if the student can't concentrate, or doesn't understand the lessons.
