If interim therapeutic anticoagulation is indicated- offer apixaban or rivaroxaban first line. If apixaban or rivaroxaban are unsuitable, consider -LMWH for at least 5 days, followed by dabigatran or edoxaban OR LMWH alongsie Vitamin K antagonist (warfarin) at least 5 days. Perform baseline blood test, FBC, U&E's, prothrombin time, and APTT. Do not wait on results before starting treatment
