Approach to a patient with generalised joint pains for 1 week

Symptoms analysis: Duration, intensity, migratory vs. fixed pain, morning stiffness.
Associated symptoms: Rash, fatigue, weight loss, photosensitivity, Raynaud’s, dry eyes/mouth.
Fever characteristics: High-grade vs. low-grade, pattern, associated chills, or sweating.
Recent illnesses: Viral or bacterial infections, travel history, exposure to sick individuals.
Medical history: Autoimmune diseases, joint disorders, medications, and family history.
Lifestyle factors: Physical activity, smoking, alcohol, diet.

General examination: Rash, oral ulcers, lymphadenopathy, signs of anemia.
Joint examination: Swelling, redness, tenderness, range of motion.
Systemic review: Heart (murmurs), lungs (effusions), abdomen (hepatosplenomegaly).

Post-infectious arthritis: Viral (chikungunya, dengue, parvovirus), bacterial (streptococcal reactive arthritis).
Autoimmune/connective tissue diseases: Rheumatoid arthritis, lupus, reactive arthritis.
Infectious causes: Rheumatic fever, tuberculosis.
Metabolic causes: Gout, pseudogout.
Others: Fibromyalgia, Vitamin D deficiency.
