Does the student exhibit the following signs:
Difficulty with basic reading, writing, spelling, and/or math skills
Difficulty paying attention/following directions?
Difficulty with staying organized?
Poor memory?
Difficulty staying on task?
Getting easily frustrated?
Taking longer to complete assignments/fails to complete assignments even when given additional time?
Becoming anxious when asked to complete schoolwork?
Yes
Differentiate within the universal classroom. This can include:
Preferential seating
Avoiding visual clutter on the board
Breaking instructions into small steps while using kinesthetic cues
Allow extra time for completing work and assessments
Provide text in large print with less on the page if possible
Does the student show improvement in response to differentiation?
Yes
No
Recommend the student to the Student Support Team (SST).
Does the SST recommend further action?
Yes
Get the parents involved. This can include the following:
Provide assistance and ideas for how the parents can help at home with homework and activities related to the curriculum
Encourage at-home reading
Work with the parents to set-up a study routine, including set times in a distraction-free environment
Provide parents with resources and strategies used in the classroom to use at home in order to provide consistency
Determine an Individualized Education Plan (IEP)
