Pathophysiology
Structural brain findings: Ventricular enlargement; Subtle reductions in total gray matter volume; Reductions in gray matter volume of the hippocampus and other medial temporal and limbic regions
Functional brain findings: Decreased activation of the prefrontal cortex; Increased activation of temporal regions during hallucinations
Neuroendocrine, Oxidative, and Immunological: Elevated markers of oxidative stress, varying by clinical status; Dysfunction of the hypothalamic–pituitary–adrenal axis; Abnormal levels of inflammatory cytokines
Neurochemical: Reduced N -acetyl aspartate in frontal and temporal regions; Reduced PME (marker of membrane phospholipid synthesis) in prefrontal regions; Elevated presynaptic dopamine function
