Swelling: Altered comfort and risk for altered perfusion related to the inflammatory response to injured tissue caused by trauma.

Pins and needles in toes and fingers: risk of neuromuscular dysfunction related to injury or treatment resulting in discomfort expressed as tingling sensation and numbness
Self care deficit: patient unable to participate in self-care and other activities of daily living like bathing, grooming, elimination, eating due to diseases or injury.
Disfigurement: Altered body image secondary to trauma, surgery and diseases process resulting in scars, contractors, poor posture, poor gait and loss of function.
Immobility: Impaired mobility related to injury , stiffness, weakness inability to bear weight or.
Expected outcome
Pain: patient verbalising that there is no pain or the pain is getting better or less intense with medication .
Immobility: range of movement of affected joint restored and maintained. Range of movement of an affected joints maintain. No sign of contractors at the joints observed.

Swelling: swelling subsided and no evidence of empire circulation distal to the legion. No sign of nerve compression.

Disfigurement: Patient having accepted or adapted to the changes in appearance. Patient participating in the rehabilitation plan.

Infection: no infection evident ,vital signs normal and wound clean and dry

Disfigurement: monitor their rehabilitation progress and encourage patient to participate in the planning their rehabilitation, activities of daily living.
