Additional Evidence to collect to further assist with investigation and inquiries
Medical History
History of eyesight checks from both involved individuals (Massey & West)
Determine if either involved parties require prescription glasses (specifically whilst driving)
Determine if further eye tests and requirements for safety whilst driving is required
Mechanical Components
Conduct inspection of brakes (both car and bike) to determine if potential failures/ errors contributed
Conduct full vehicle inspection to determine if mechanical failures are present and likely contributors
Conduct inspection of bike and determine if mechanical/ functional errors are present
Current Health Condition
Blood sugar levels of diabetic (Massey)
Determine test results if high blood sugar levels were experienced and confirm if blurred vision, tiredness/ fatigue are contributing factors
Determine test results if low blood sugar levels were experienced and confirm if contributing to dizziness or reduced attentiveness
Conduct full testing on syringe found in vehicle- was it insulin or another product
What do lab results reveal?
Was there any prescribed medical paperwork?
Was there a history of other medical conditions?
Should a medically approved/ professional health review be conducted on both parties to ensure driving is safe?
Was the thick red fluid professional tested by a lab? Does it reveal who's blood it was and is this a match to the involved pareties?
Were the involved parties shift workers - did fatigue or time pressures contribute to lack of awareness or other health related capacities?
CCTV Footage/ Physical Area
Determine if additional camera footage/ material is available that captured the incident (prior to, during, and after)
Identify if other witnesses were in the area at the time of the incident
Conduct full review of dash cam footage and analyse specific incident events- was there discrepancies in witness statements?
Review road markings and traffic signage- is it adequate for the area or are improvements required?
Should a pedestrian crossing be implemented or additional signage?
PPE/ Safety
Condition of Cyclist Helmet when incident occurred. Was it old and worn/ past manufacturers expiry date?
Driver/ Cyclist Sunglasses - were they scratched, cracked/ faded tint, too tight or loose?
Bicycle condition - was it full functional and inspected? Grease/ oil on brake wires etc.
Service history documents - were there any missed items or notes for consideration that potentially contributed to the accident?
External Considerations - was this vehicle/ bike brand a known model for accidents and mechanical errors?
Are there any systems of work documents that can be reviewed?
