What's wrong with BK?
Something is visibly and outwardly so wrong with him that the parents can't take him to an adoption agnecy

Who is BK?
The parents' first biological child is BK, but after BK was injured in a terrible accident, they locked BK up in the basement to hide the "embarrassment" of a disfigured child and raised their normal, second child upstairs.
The parents' only biological child died, and they attempted to clone that kid. They had two viable clones and UK was slightly better than DK. They kept DK locked up because at-home genetic engineering is illegal.
The parents' real kid, UK, needed organs, so they kidnapped BK and used him as an organ farm.
The parents' real kid is BK, but after he became disfigured or disabled, they got a new adopted kid, UK.
Maybe there is a more hard sci-fi element. What if the parents can literally take UK's woes and place it onto BK, like the Omela in LeGuin's story? But I'm a little iffy about sci-fi.
