Nursing diagnosis
Low self esteem due to physical factors, hormonal changes and/or loss of reproductive capability evidenced by depression, nervousness, being negative about herself and/or expressions of guilt.
Possible outcome
Improving of self esteem and self worth, decreasing of nervousness and depression.
Nursing intervention
Encourage patient to express her feelings to anyone that she can trust.
Tell patient that all these emotional feelings are caused by reduce in secreation of hormones and are usual, to make her to understand her problem so that she can be able to accept.
If she doesn't improve then refer her for counselling to prevent extension of depression that may end up causing chronic emotional disability.
Nursing diagnosis
Sexual dysfunction related to changes in body structure and function from decreased oestrogen secretion as evidenced by dyspareunia.
Possible outcome
Patient will have satisfying sexual function.
Nursing interventions
Assess patient's sexual desire desire and psychosocial factors affecting sexual function.
Include patient's partner if possible so that he/she can understand patient's sexual situation and to avoid putting pressure towards her.
Discuss past sexual experience and satisfaction so that you can be able to give individual needs on sexual behavior based on history.
If patient is still sexual active encourage her to use lubricants in order to prevent dryness tha can cause pain.
Ask patient to do some Kegel exercises everyday to strengthen vaginal and pelvic muscles.
Nursing diagnosis
Disturbance in sleeping patterns due to insomnia and stress as evidenced by hot flashes, anxiety, depression and insomnia.
Possible outcome
Patient will be able to follow correct sleeping pattern that are normal and express feelings of being rested.
Nursing interventions
Assess patient's sleeping patterns so that you will be able to provide sleeping techniques that can help patient.
Ask patient to rate her discomfort from 0 to 10 and tell her about all factors that can cause frequent awakenings helps to facilitate changes in sleep patterns.
Put her on quite environment with closed curtains and adjusting lighting.
Administer medication of promoting normal sleep patterns as prescribed.
Ask patient to avoid the use caffeine like drinking coffee,reboost e.t.c because they disturb good sleep pattern.
Ask patient to avoid alcohol intake because alcohol may lead to sleeping during intake, and interrupt sleeping at night.
