Red itchy rash
Pain
Burning
Numbness
Tingling
Fluid filled blisters
Nursing intervention
Wearing of loose clothes
Application of cool, moist dressing to puritic lesions
Avoid rubbing or stretching the patient skin
Administration of medication as prescribed
Nursing assessment
Assess the presence and location of skin lesion
Assess the signs of pain
Ask the patient iif she has any pain or discomfort
Assess for the history of shingles
Expected outcomes
Patient will remain free of infection
Nursing evaluation
Patient will report comfort evidenced by enough rest
Patient will demonstrate the positive body image
Minimal risk of disease transmission
The patient will maintain defenses
