onychauxis is a nail disoderd that causes finger nails or toenails grow abnormally thick , overtime the nails may become curled and turned white or yellowish

nursing care plan
altered comfort and disturbed sleep due to pain caused by separation of the nail plate from the nailbed
-risk for infection related to separation of nail plate from nailbed.
-patient verbalize that there is no pain
-patient to get 6 to 8 hours of sleep without being disturbed
-No signs of infection
-vital signs are normal
-
-administer analgesic as prescribed
-cut nails short
-administer antibiotics , antifungal or antivirals as prescribed
-monitor vital signs
-advice the patient to eat a diet high in vegetables and fruits to fight infections
