How does student get to school, if by city bus what are the routes
Have they seen the IEP and IPRC and can they make necessary accommodations
Is a social worker present on site
contact information, who can student and parents connect with
How consistent will communication be regarding progress of student's transition
How long does it take student to get there
Meet with guidance to go over course options and timetable
Is assistive technology needed, is it available, if not when does SEA application need to be submitted so it arrives when school year begins
What stream will the student be placed in
What programs are available: Are there antyspecialized programs, tech emphasis, arts etc.
Are they any extracurricular activities student can join in
What social and community supports are available
Has administration met with student and family
Has adminstration communicated with previous school regarding students needs
What happens if a student is struggling with content even with accommodations, what the the next steps
What is the school population size
What is the start time, end day, transition times between classes and lunch time
What support services are available
Does the school have all the assessments needed from IEP, IPRC or other medical professional vital to student's success
Do they have the student's safety plan or communicate with the student if they are overwhelmed what they do to calm down
DO they have EA, LST, SERT and student success/resource room available
What teachers have successfully supported Spec Ed students
What classes are available that meets the strengths and interests of the student
Aware of any potential social-emotional, family factors that can affect student at school
Are the expectations of the school outlined
Are there any peer leadership supports available
Can a school tour be set up before the school year so student can get to know ahead of time where classes, offices, cafeteria, gym, student success, social worker and other areas that they will use are
What is the average student to teacher ratio
What is the school's plan to enable student can access EA's, SERT LST or have the technology student needs
How successful was student in achieving credits and are there any credit gaps
What courses did they do well in
What are the student's long term academic goals
What does the student plan to do post-secondary
Are there any compulsory courses and is this a year or provincial testing
