Teacher contacts school counselor
If needed, Counselor recommends starting an SST
SST meets with teacher, principal, counselor and any other teachers involved
Counselor starts IEP and sends it out to all teachers involved
Teachers input used strategies into IEP
SST finalizes the IEP and sets up meeting for teachers and parents.
Parents and Teachers meet to discuss steps moving forward including possible testing
Principal notifies parents that SST has been formed to discuss needs of student
Teacher tries differentiation
Teacher conferences with student
Teacher meets with principal
Student is at grade level, no action needed
Student needs extra scaffolding or differentiation, but no referral to special education team
Teacher Receives No Performance Data
Teacher performs formative assessments in class to gauge student knowledge
Student is at grade level, no action needed
