Upper Gastrointestinal Bleeding
สาเหตุ
ภาวะเลือดออกทางเดินอาหารส่วนต้น
(upper gastrointestinal bleeding, UGIB) เป็น
ภาวะเลือดออกในทางเดินอาหารส่วนต้นที่ตำแหน่ง
สูงกว่า ligament of treitz ประกอบด้วยหลอดอาหาร
กระเพาะอาหาร และลำไส้เล็กส่วนดูโอดินัม
เส้นเลือดโป่งพอง (variceal bleeding)
ได้แก่ rupture esophageal or gastric varices,
portal hypertensive gastropathy
ไม่ใช่เส้นเลือดโป่งพอง (non-variceal
bleeding) ได้แก่ peptic ulcer, erosive gastritis,
tumors, reflux esophagitis, Mallory-Weiss
syndrome, Dieulafoy’s lesion และ angiodysplasia
อาการ
มีอาการปวดท้อง
อาเจียนเป็นเลือดสด (Hematemesis)
ถ่ายอุจาระเป็นสีดำ (Melena)
ซีด อ่อนเพลีย เหงื่อออก
ปวดศีรษะ หน้ามืดเหมือนจะเป็นลม
มือเท้าเย็น เป็นตะคริว
ชีพจรเบาเร็ว ความดันเลือดต่ำ ใจสั่น
การรักษา
1.NG irrigation
2.IV Fluid
3.Blood transfussions
4.Omeprazole or Pantoprazole
5.งดสูบบุหรี่และงดดื่มแอลกอฮอล์
6.Surgery
