SKIN DISEASES IN THE TROPICS
DENGUE
Pathogen: Dengue virus (DENV)
Vector: Aedes aegypti or Aedes albopictus
Clinic Manifestations
Dengue without warning signs
Nausea, vomiting, rash, headache, eye pain, muscle ache or joint pain, leukopenia, positive tourniquet test.
Dengue with warning signs
Abdominal pain or tenderness, persistent vomiting, clinical fluid accumulation like ascities, pleural effusion, mucosal bleeding, lethargy or restlessness, hepatomegaly >2cm, increase in hematocrit concurrent with rapid decrease in platelet count.
Severe dengue
Shock, fluid accumulation witht respiratory distress, severe organ involvement, severe bleeding, AST o ALT>1000, impaireed consciousness, organ failure.
YELLOW FEVER
Pathogen: Flavivirus
Vector: Aedes aegypti, Haemagogus y Sabethes
Clinic manifestations
Period of infection:
fever, generalized malaise, headache, photophobia, lumbosacral pain, pain in the lower extremities, myalgia, anorexia, nausea, vomiting, restlessness, irritability n dizziness.
Abortive, nonespecific febrile illness without jaundice
Life threatening disease with fever, jaundice, renal failure and hemorrhage
Subclinical infection
Flushed skin, redding of the conjuctivae and gums, epigastric tenderness, Faget's sign.
Leukopenia, transaminases rising, liver enzyme abnormalities.
CUTANEOUS LEISHMANIASIS
Pathogen: Leishmania
Vector: Sand fly
Clinic manifestations:
Pink-colored papule that enlarges and develops into a nodule or plaque-like lesion (often with central softening)
Multiple lesions, nodular lymphangitis, secondary bacterial infection.
