Definition: predetermined set of questions designed to capture data from respondents

In person requires face-to-face contact with respondents

Over the telephone (personal interviewing which is used to obtain information quickly)

Electronic (administered via the intranet or internet through the use of email)

Questionnaire design

everybody should understand the the question in the same manner

each question should deal with only one dimension or aspect

no unrealistic demand should be made on the respondents' know-how, memory and willingness to respond

no escape route in the question

questions must be asked in simple and concise language

questions should be formulated in polite and soft language

layout of questionnaire should look neat and tidy

re-check the questionnaire and pre-test

only sufficient items should be included to cover all important aspects of inquiry

Validity and Reliability

At least one other person should read the questionnaire thoroughly before it is finalized, which would reflect an independent point of view
