Root causes of underperformance
The NAO identified some root causes of projects underperformance:
were initiated without an understanding of feasibility or delivery
had weak governance structures and performance management
lacked funding certainty
had project teams which poorly articulated their priorities and requirements.
Early decision making and robust project initiation are the key factors in successful delivery of a project
They can do this by aligning the capability of the project's delivery organisations to the degree of complexity posed by the unique project or programme and its environment.

The Delivery Environment Complexity Analytic (DECA)
provides a rapid appraisal of challenges, complexity and risks to delivery
It provides a complexity profile that you can use to focus discussion and effort.
By being informed about how complex your project is, you can then take action to shape the delivery environment into one that promotes more successful outcomes and supports the major project's objectives.
How and why did the National Audit Office create the DECA?
The DECA assesses complexity across twelve factors. Each has an impact on the successful delivery of objectives and outcomes, and so these factors can predict major project performance.
