Accident Paperwork Process
Urine Analysis (UA/Drug test/ pee test) (for most accidents)
Call Amit/Maya with TenaCare to schedule the UA.
Needs to happen ASAP
PRINT the consent form and the UA policy
Give copy of the consent form to the worker
Scan the consent form and save to the server
Update the SmartSheet with the employee info
Email name/s to Maria Tena
If there is an Injury that goes off-site for care- The ACCIDENT PACKET must be completed- Due the same day or the next morning
The accident packet is supposed to be completed and sent to Sheri Sundstrum, John Chriswell, Jennifer Dorner.
AON/Employer Liability Form is sent to the Intel/Kennedy Law Contacts.
Save on the server (Accident packets, then company)
Any dates need to be written out (Due to Americans being American)
KEYLEE ACTION ITEMS
Create form for the subcontractors to give information that is needed on the AON Insurance form. (employee details, contractor contact info, etc.)
PPT Training for subcontractors to complete the accident paperwork
Add the injury to the first-aid log (Incident-Accident Reports- First Aid- Myles- Sparrow_Safety) on the server
Ensure that we are using one of the pre-defined "root causes" on the spreadsheet
Information added should mimic what is already on the spreadsheet (i.e. the date written with the day of the week). A lead should be helping complete this.
If an injury is anything other than a first-aid, we will let you know.
Complete the EROI
The EROI is one SharePoint, under Departments, Risk Management, Electronic Reports of Incident, Injury, etc.
Utilize the report of incident from the accident packet- ensure that everything written is as accurate as possible. Sometimes there is subjective information in the reports we get from subcontractors.
For example "he seemed to be faking his injury" "it didn't look like he was hurt that bad" etc.

UA's should be done first, after care is given to the injured worker- IF a UA is requested at all. Rick Jenkins and Myles Byrd make the decision on whether or not a worker needs to be drug tested.

FIRST AID
Employee receives treatment on site. And no further treatment is required.
Step 2: First-aid log must be updated with details of the injury. (THIS MAY GET TAKEN OVER BY TENACARE)
