Nursing diagnosis
Inaffective airway clearance related to swelling evidenced by difficulty swallowing and breathing with mouth
Possible outcome
Patient will be able to swallow without any difficult
Patient will maintain a palent airway as demonstrated by normal respiratory rate of 18 to 22bpm, rhythm and clear breath sound
Nursing intervention
Place patient in prone or side lying position to promotes drainage of blood and unswallowed salva from mount that can be potentially aspirated
Give soft diet to patient
Demonstrate breathing exercises to promotes lung expansion
Administer medication as prescribed
Inserting Nasogastric tube if ordered
Nursing diagnosis
Risk for deficient fluid volume related to inadequate oral intake evidenced by poor fluid intake
Possible outcome
Increased urine output to normal ranges of 0,5 to 1ml per kg per hour
Nursing intervention
Measure and record intake and out put hourly
Assess skin turgor to see if there's no dehydration
Monitor viatl signs especially blood pressure and pulse
Administer IV fluid as prescribed
