Nursing intervention
Give accurate, honest information
Discuss the probability that careful monitoring and treatment can prevent additional vision loss
Allow patient to acknowledge concerns and express feelings
Identify helpful resources and people so that patient will know that is not alone in dealing with problem
Nursing diagnosis
Disturbed visual sensory perception related to altered status of sense organ evidenced by progressive loss of visual field
Desired outcome
Patient will participate in the therapeutic regimen
Patient will maintain the current visual field or acuity without further loss
Nursing intervention
Allow patient to express feelings about loss and possibly vision loss
Demonstrate administration of eye drops to control IOPs, preventing further loss of vision
Stress the importance of meticulous compliance with prescribed during therapy to prevent an increase in IOP resulting in disk changes and loss of vision
Provide sedation and analgesia as prescribed
Determine type of degree of visual loss because it affects choice of patient's future expectation
Nursing diagnosis
Dificient knowledge related to lack of recall information, information misinterpretation and lack of exposire evidenced by development of preventable complications and inaccurate follow through of instruction
Desired outcomes
Patient will verbalize understanding of treatment needs
Patient will correctly perform necessary procedures and explain reasons for the action
Patient will verbalize understanding of condition and prognosis
Nursing intervention
State the importance of glaucoma screening
Review pathology and prognosis of the conditio and lifelong need for treatment
Discuss the necessity of wearing identification
Encourage the patient to make necessary changes in lifestyle i.e. decrease emotional response to stress
