Hydrocephalus
Signs and symptoms
Nursing care plan
Pathophysiology
Definition
Memory loss
Unable to control bladder
Headache
Muscle problem
Loss of appetite
Lethargy
Unusual large head and increasing head size
Caused by an imbalance between CSF produced andthe one distributed in bloodstream
Too much CSF in ventricles ocvurs because of obstruction, poor absorption and overproduction
The accumulation of CSF can cause head enlargement
Fluid buildup in the ventricles within the brain
Increase pressure of CSF in the brain and spinal cord can damage brain tissues causing dysfunction of the brain
Nursing diagnosis
Nursing interventions
Possible outcome
Ineffective cerebral tissue perfusion due ro decreased venous and arterial blood flow and increasd intracranial pressure possibly evidenced by enlarged size of the head, impaired blood flow
Patient will have no pressure of the cranial
Assess vital signs hourly and observe for any signs if dsypnea and change in pulse rhythm
Assess neurological satus e.g motor, sensory, eye movement and mental state
Examine pupil for change in size and shape
Note abnormal crying episodes
Measuring head circumference and appearing of fontanelles
Provide oxygen therapy if needed
Administer medication as prescribed
