pathophysiology
As the cataract continues to develop, the clouding becomes denser and involves a bigger part of the lens. A cataract scatters and blocks the light as it passes through the lens, preventing a sharply defined image from reaching the retina. As a result, the vision becomes blurred.
The lens, where cataracts form, is positioned behind the colored part of the eye (iris). The lens focuses light that passes into the eye, producing clear, sharp images on the retina ,the light-sensitive membrane in the eye that functions like the film in a camera

Check for any signs of infection, discharge or foreign objects in the eye

Vision loss due to disease process caused by cataract,as evident by cloudy iris of the eye

Psychosocial information: enquire on patient's lifestyle, socioeconomic circumstances, medication history
objective data
Observe for any foreign objects, swelling,scarring, any discharge and the color of the eyelids.
check if there are any injections, foreign bodies, chemosis and discharge on the Conjuctiva
On the pupils, observe the shape, the size, and the reaction to light.
