Case 18: Chemical Pathology (Endocrine Causes of Hypertension), NOTE: If…
-
NOTE: If the Blood Pressure is Elevated and the Serum Potassium is Low
- Find out the Concentration of urine Potassium (K+)
- If the Urine Potassium is Low (less than 30 mmol/day) then suspect Extra-Renal cause of Low Potassium (GIT Loss)
- If the urine Potassium is High (more than 30 mmol/day) then Find out the Serum Concentration of Renin
- If the Renin Level is High and the Concentration of Potassium in the Urine High then think
- Renal artery stenosis
- Renin-producing Tumour
- If the Renin Level is Low and the Concentration of Potassium in the Urine is High then ask about the Aldosterone Levels
- If Aldosterone Level is High, the Renin Level is Low and the Potassium ion concentration in the urine is High then think:
- Primary Hyperaldosteronism (Conn's Syndrome)
- If the Aldosterone Level is Low, the Renin is Low and the Potassium ion concentration in the Urine is Low then think:
- Cushing's Syndrome (Glucocorticoid Excess)
- Defect of the 11-Beta-Hydrosteroid Dehydrogenase Type II Enzyme
- Liddle Syndrome
