Atresia
Intestines are malformed
aspiration, drooling, doesn't tolerate feeding, instant from birth. Bilious vomiting
Stenosis
Narrowing of the small intestine
Can take longer to present. Prolonged vomiting and dehydration, low weight, electrolyte imbalance
Meconium Ileus
Thicker meconium than usual due to disease, typically CF.
Abdominal Xray - soap bubble or ground glass bowel
Tx- enema or bowel resection
Hirschsprung
Malrotation
Congenital Bands
idiopathic congenital adhesion
Internal Henria
Duplications
Small Bowel Obstruction
Adhesions
Surgical History (2 weeks- 30 yrs ish)
Strictures
Fibrotic Stricture
Bilious Vomiting
Partial Obstruction
Associated with Crohn Disease
Gallstone Illeous
Gallstone creates fistula to the illeous from gallbladder,
Ascariasis
Peripheral Eosinophilia
Travel from Asia, Africa, S Ameria
Cough, Pneumonitis (1-2 wk)
Abdominal pain, anorexia, diarrhea (1-2 months)
=SBO, Biliary Colic, Cholangitis, or Pancreatitis
Hernia
Non-reducible bulge with Valsalva
Erythema, Warmth, Nausea, Vomiting
FEMORAL high chance
Malignancy
Large Bowel Obstruction
Volvulus
Risk: Constipation or underlying neurologic d/o
Slowly progressing discomfort and distension
U shaped Loop, Coffee Bean Sign
TX: Endoscopic detorsion (flexible sigmoidoscopy)
elective sigmoid colectomy or emergent
Recent Surgery
Paralytic Illeous
CT - diffuse air, distension through entire bowel
HypoKalemia or Hypophosphatemia can also cause this due to muscle dysfunction
HPI
Crampy Pain
High Pitched Bowel Sound
