Pathophysiology
in the respiratory infections, infective organism ascends along the eustachian tube into the middle ear
Patient may have an antecendent events usually in the upper respiratory viral infection resulting in the congestion of respiratory mucosa including nasopharynx & eustachian tube
Inflamation reaction which is caused by respiratory infection in the eustachian tube become swollen and blocked resulting in a negative pressure in the middle ear
Fluid stagnates hence there's no drainage , osscicles movable jointt becomes immovable and therefore the will be no transmission of sound waves .

In the presence of pus the eardrum bulges outwards with a possible risk of perforation ,if the eardrum perforate there will be a prulent discharge from the ear
Nursing management
administering of prescribed medications such as analgesics in the form of antibiotics like amoxycillin and paracetamol in order to relieve pains and exclude infections
- if there's any discharge in the ear then the ear should be kept dry by wicking the ear
Health education
Encourage the patient to take prescribed analgesics for pain and ensure the he/she finish the course prescribed by doctor to clear the infection
Inform the patient that if the discharge is not clearing after 14 days then he/she be reffered for further management

Nursing care plan
Administer pain medication such as paracetamol and amoxycillin and ibuprofein as per doctors prescription

Monitor and record vital signs closely hence a normal response to pain is an increased respiratory rate , heart rate and blood pressure, fever may cause discomfort
The patient must sit up, put pillows behind the head or lie on the unaffected ear to promote drainage and reduce pressure from fluid
