incontinence - she cant get to the toilet on time because she forgets to use her walking frame. isolated . This may result in a fall or injury. promote independence by giving Audrey regular reminders to use the toilet - so she doesnt spoil herself dignity and respect
weight gain can cause strokes, heart disease, diabetes we can reduce this by putting in physical activities such as gardening, walking and dance classes.
The staff need more continued professional development to understand where they went wrong to prevent the same situation from happening again.

All patients must presume to have capacity , they must be supported to make decisions

trust and safeguarding issue- because she has broken the policies and procedures regulations - has not declared that she has taken her medication danger of overdosing her and lack of communication or staff not recording abuse
if audrey is bed bound she could be at risk of diabetes arthiritis bed sores , to prevent this we could ask audrey to do exercise - early intervention and prevention
duty of care and duty of conduct is a framework which enables health care workers to follow guidelines and treat patients with dignity respect putting medication in her food without gaining consent
