Heart failure
-
-
-
Loop diuretics: furosemide, torsemide, butmetanide Suffix: -ide
-
ACE inhibitors: captopril, enelapril, lisinopril Suffix: -pril
-
Angiotension 2 receptor blockers (ARBs): losartan, valsartan suffix: -sartan
MOA: vasodilators that decrease systemic vascular resistance (afterload), used with diuretics for treatment of HF
Side effects: upper respiratory infections; hyperkalemia much less likely to happen than with ACE inhibitors
-
Beta Blockers: metoprolol, carvedilol, labetalol, atenolol, asmolol, propranolol Suffix: -lol
MOA:Block SNS stimulation to heart - reduce BP and HR, delay conduction, reduce myocardial contractility - used for diastolic heart failure
Side effects: hypotension; thrombocytopenia; possible heart failure; mental depression; ischemic colitis; possible bronchospasm if non-selective beta-blocker given. can mask signs of hypoglycemia
-
PT teaching: rebound hypertension may occur if medication is discontinued abruptly. tell pts to report weight gain, SOB as beta-blockers may precipitate heart failure. take apical pulse daily. administer IV form slowly
contraindications: non-selective beta-blockers may cause respiratory depression in pts with asthma, COPD
