Dyspnea, - Coggle Diagram

- - - - Normal shape, size and movement, AP Transverse 1:2, no sternal contraction and barrel chest. Normal breathing movement. Lung sound Crepitation both lung. no chest pains.       - การดู โครงสร้างรูปร่าง ความกว้างของทรวงอก (AP Transverse 1:2) จังหวะการหายใจเร็ว อัตราการหายใจเร็ว บางครั้งมีหายใจลึก
      - การคลำ หลอดลมใหญ่ ทรวงอก ปกติ รวมถึงการขยายของทรวงอกและปอด มีความสัมพันธ์กัน ไม่สามารถประเมินการสั่นสะเทือนของเสียงสะท้อนได้ เนื่องจากผู้ป่วยใส่ท่อช่วยหายใจ
      - การเคาะ ไม่สามารถประเมินได้ เนื่องจากผู้ป่วยใส่ท่อช่วยหายใจ
      - การฟัง เสียงการหายใจ ผ่านหลอดลมเบา แต่เสียงปอดมี Crepitation ของปอดทั้ง 2 ข้าง ไม่ได้ยินเสียงสะท้อนหรือเสียงสะท้อน เนื่องจากผู้ป่วยใส่ท่อช่วยหายใจ
  - - - Normal posture and chest configuration, no visible cardiac movement, no neck vein engorged, no short of breath, no cyanosis, no clubbing fingers, and แขนและขาบวม pitting edema 1+ Color appropriate for race, feel warm bilateral, capillary refill time > 2 second. Apical impulse with regular rhythm in 5th medial at intercostal space medial at mid-clavicular line, appropriately 2 cm. in diameter, no trills, heaves, or rib retraction. LBCD (Left border cardiac dullness) at mid-clavicular line in 6th intercostal space. EKG normal sinus rhythm และพบ Ventricular premature contraction, Heart rate 88-100 min. No murmur. Normal S1S2
      - Chest X-ray = Cardiomegaly
      - การดู ไม่สามารถมองเห็นการเต้นของ Carotid artery และ Jugular veins ได้
      - การคลำ สามารถชีพจรได้ ปกติ อัตราการเต้นของหัวใจ 88-100 /min การคลำบริเวณปลายยอดของหัวใจอยู่ที่ Mid-clavicular line in 6th intercostal space เนื่องจากหัวใจของผู้ป่วยโต
      - การฟัง เสียงหัวใจ No murmur. Normal S1S2
- - - - เกิด Left side heart failure
      - Pulmonary hypertension ส่งผลให้เกิด Pulmonary venous congestion เกิด Lung sound crepitation
        
        ส่งผลให้เกิด Respiratory failure type 1 เกิดการเเลกเปลี่ยนก๊าซไม่เพียงพอต่อความต้องการของร่างกาย เกิด Hypoxia เเละ Hypercarbia
        
        ผู้ป่วยเกิดอาการหอบเหนื่อย นอนรายไม่ได้ (Dyspnea)
        
        ผลการตรวจของผู้ป่วย PaO2 = 60 mm.Hg และ PaCO2 50 = mm.Hg
