Progestin only
Special Populations: use in women 35 yo and older who smoke (high risk of MI); Smoking 15 or more cigs per day is contraindication to CHC --> use progestin-only or nonhormonal contraception
MOA: progesterone only "minipills"; 28 days of active hormone; contraceptive effect contributed to thickening cervical mucus to prevent sperm penetration, slowing tubal motility, delaying sperm transport, and inducing endometrial atrophy. Progestins block the LH surge, therefore inhibiting ovulation
Adverse effects: irregular and unpredictable menstrual bleeding, may not block ovulation so risk of ectopic pregnancy is higher with their use than other OC
Counseling: take every day at the same time of day; if taken 3 hours late, use backup contraception for 48 hours
