Spine pain
Mechanical or Somatic
Most common 85-90%
Varies with activity and time
Rarely severe at rest or nocturnally
Person is well without red flags
May radiate in non-dermatomal pattern
Stiff and sore in morning but improve once up and moving
Aggravated by too much activity
NSAIDs will help to some extent
Many patients settle from an acute episode in 6wks
Disc pain
Back pain with no leg pain
Discogenic
Damaged/ degenerative IV disc without nerve root involvement
No radiation
Responds poorly to NSAIDs
Woken at night by frequently by persistent pain
Stiff and sore in mornings
Gets worse with activity
Back pain + leg pain
Radicular pain
Disc injury with prolapse or inflammatory response involving nerve root
Leg pain > back pain
Arm pain > Neck pain
Often radiates below the elbow and knee
Dermatomal pattern
Radicular pain
Spinal pathology
Visceral pain referred to back
