Client at increased risk of confusion, headache, and seizure related to hyponatremia.

Refer client to dietician for nutrition support. Following consult, work with client to ensure adherence and understanding of new diet.

Client Education
Educate the client about the potential causes of hyponatremia including: excessive sweating, vomiting, diarrhea, use of diuretic drugs, low-sodium diet, excessive water intake, and/or hormonal imbalances (Aldosterone/ADH).
Educate the client on the basics of sodium's role in the body and the importance of salt in their diet (consult dietician as necessary).
