Endodontic treatment is necessary if oral disease or trauma involves the pulp of the tooth such as if it is infected and destroyed by caries, periodontal disease, or trauma.

Class II
Cavities located in the proximal surfaces (mesial and distal) of posterior teeth (molars and premolars).
Class III
Cavities in the proximal surfaces of canines, and incisors not involving the incisal angles.
Class I
the occlusal surfaces of molars and premolars, the occlusal two thirds of the buccal and lingual surfaces of molars, the lingual surfaces of anterior teeth
Class IV
Cavities in the proximal surfaces of incisors or canines which also involve one or both of the incisal angles.
Class V
Involve the cervical third of all teeth, including the proximal surface of posterior teeth where the marginal ridge is not included in the cavity preparation (Do not involve pit & fissure areas)

Definition
Dental caries is a concentrated chemical dissolution (demineralization) of a tooth surface by a metabolic activity in a microbial waste deposit (a dental biofilm) covering a tooth surface at any given time. The dental biofilm (called dental plaque) is a microbial biomass composed of bacteria from saliva.
