Teacher or parent (really any stakeholder who works with the student) who has recognized areas of struggle, sends a request for observation to the Learning Support Team

TIER 1 Learning support provides strategies to the teacher for them to implement themselves. The teacher should let the family know of their concerns and keep anecdotal notes of the classroom changes that are being implemented.
Struggles continue, concerns persist! Again this can be from any stakeholder in the student's life!
A meeting is held with the classroom teacher, Learning Support Team members, school counselor and the principal to discuss the next step in support for the struggling student
Stay the course! Give the interventions and strategies a few more days or weeks to work with the student!
