Patient will report back for follow up in 3 months. in that time the patient will record daily blood pressures to identify the reasoning for the suggested treatment plan

client will be asked to research and report back 5 health related risk factors for individuals with uncontrolled hypertension before discharge. this is so the client can gather the information from their own sources to prevent any mistrust in information given from the provider.

Client will adhere to taking medications as prescribed for 3 months. in that time the patient will record the number of headaches she experiences per week

Client will identify both modifiable and non-modifiable risk factors associated with hypertension before discharge

Interventions

our interventions will work to promote learning from the client’s own actions, then identify those actions that have caused us to become defensive or cope negatively.

By identifying risk factors associated with the patients condition we can individualize our care plan and allow the client to see how their own behaviors are leading to the condition they are in

Reinforce more adaptive coping patterns (like exercise, dieting, medication and diagnosis education) that assist the person in achieving our established plan

Support for self-management. We are going to encourage the client to be independent when it comes to their care, and encourage them to take responsibility for good and bad healthcare practices
