Diabetes- a chronic multisystem disease characterized by hyperglycemia from abnormal insulin production, impaired insulin use, or both.
Type 1 5-10% of Diabetes
-
-
autoimmune disorder in which the body develops antibodies against insulin and/or the pancreatic B-cells that provide insulin.
-
-
-
Complications
Acute
Hyperglycemia
-
causes
-
-
-
-
-
emotional, physical stress
-
-
-
-
-
Hypoglycemia
symptoms
-
-
numbness of fingers, toes, mouth
-
-
-
-
-
unsteady gait, slurred speech
-

treatment
Rule of 15: 15 g carbs, recheck glucose after 15 min. if value still < 70mg/dL continue every 15 min until above

Chronic
Angiopathy
Macrovascular
disease of large and medium blood vessels that occur with greater frequency and an earlier onset in people with diabetes
-
Microvascular
result from thickening of the vessel membranes in the capillaries and arterioles (small vessels) in response to chronic hyperglycemia
retinopathy
-
nonproliferative
most common; partial occlusion of the small blood vessels in the retina causes micro-aneurysms to develop in capillary walls
-
-
-
neuropathy
-
-
-
-
autonomic
-
can lead to hypoglycemia unawareness, bowel incontinence and diarrhea, and urinary retention
-
-
-
Diagnostics
-
-
2hr plasma glucose level >= 200mg/dL (11.1 mmol/L) during an OGTT, using glucose load of 75mg

Somogyi Effect
high dose of insulin at night causes a decline in blood glucose; in response counterregulatory hormones (glucagon, epinephrine, GH, cortisol) are released. They stimulate lipolysis, gluconeogenesis and glycogenolysis, which causes rebound hyperglycemia in the morning
-
if hyperglycemic in morning have pt check BG between 2-4 am; if hypoglycemic then this is due to somogyi effect
-
-
Assess insulin dose, injection site, and variability in the time of meals and insulin administration
Dawn Phenomenon
characterized by hyperglycemia present on awakenings; two counterregulatory hormones (GH and cortisol), which are excreted in incr. amounts early morning may be the cause
-
-
if hyperglycemic in morning have pt check BG between 2-4 am; if BG is high then this is due to Dawn phenomenon
