How would you address grooming when the patient is having difficulty with fine motor coordination and vision? How would you adapt or grade the activity?
For a patient with visual deficits, I would recommend using grooming tools that contrast with their backgrounds so that they are easily identified. An intervention that could address the client's fine motor and object manipulation deficits is to introduce a hairbrush with a loop handle.

Standardized & Non-Standardized Assessment: Jade is having changes in her vision. She is having blurred vision. For this problem the assessment for vision acuity will help us to determine how her vision could affect any ADLs. Jade reports to have difficulties when feeding and grooming. The assessment Katz ADL is a great tool to measure Jade's ability to perform ADLs independently. Due to her condition, Jade is having difficulties to maintaining balance. Berg Balance Scale is a great assessment that can provide us acute information about how Jade's balance is when performing functional tasks everyday life. Observation and an interview are important tools that will give us information about Jade's medical condition.
