Pain system
-
-
Pain receptors = Nociceptors (Free nerve endings found in skin, muscle, joints, bone and viscera)
-
Different category bc not uniformly sensitive:
- High threshold mechanonociceptors = Intense mechanical stimulation (pinching, cutting, stretching)
- Thermal nociceptors = As above + thermal stimuli
- Chemical nociceptors = Respond to only chemical substance
- Polymodal nociceptors = 3 types combined
- Silent nociceptors= Become activated to mechanical stimulation during: Inflammation and after tissue injury
These are responsible for Hyperalgesia, central sensitization and allodynia
-
Pain fibers
First and second pain
(carried by 2 different primary afferent axons)
- First: Early perception of sharp, prickly pain
- Second: Later sensation, duller, burning
-
2nd (dull) Pain
C fibers --> Brainstem & Thalamus (medial nuclei) --> Ventral medial Forebrain (affective dimensions)
-
Nerves fibers types:
- A𝛿: Smaller, fast transmitting, myelinated, transmit sharp pain
MechanoR: mechanical pressure, intense T°
- C: Smallest, slow, unmyelinated, transmit dull or aching pain (70%)
MechanoR
ChemoR: triggered by chemical released during inflammat°
- A𝛽: Large diameter, fast transmitting, myelinated sensory fibers
Contribute indirectly to normal perception of stimulus quality
-
Pain sensory information
Allodynia
- Pain results from stimuli that normally are innocuous
- Patient do not feel constant pain if no stimulus
