Cushing Disease: Cushing syndrome is characterized by the onset of clinical abnormalities linked with glucocorticoid excess as a result of excessive adrenal cortisol production or long-term glucocorticoid treatment.
Cushing's disease is Cushing syndrome caused by pituitary adrenocorticotropic hormone (ACTH) excess.
-
Etiology:
-
Cushing's syndrome can be caused by overuse of cortisol medication, as seen in the treatment of chronic asthma or rheumatoid arthritis (iatrogenic Cushing's syndrome), excess production of cortisol from a tumor in the adrenal gland or elsewhere in the body (ectopic Cushing's syndrome) or a tumor of the pituitary gland
Nursing interventions:
Decrease risk of injury: provide a protective environment to prevent falls and fractures and recommend foods high in protein, calcium and Vit D.
Decrease risk of infection: avoid unnecessary exposure to infected people and asses for signs of infection.
-
Encourage rest and activity: encourage moderate activity to prevent complications and plan rest periods to promote good rest and sleep.
Promoting skin integrity: use meticulous skincare to avoid traumatizing fragile skin and assist patient to change position frequently.
Improve body image: discuss the impact on patients self concept and relationship with others and suggest about diet and lifestyle modifications.
Improve thought process: discuss impact on mental health and encourage patient to verbalize feelings and concerns.
Treatment:
The goal of treatment is to rapidly control cortisol excess and achieve long-term remission, to reverse the clinical features and reduce long-term complications associated with increased mortality.
